In practice, this happens almost never. The only problems I ever saw 
were the old diff format and the broken supercite attribution line.

huh? It's a grey bar for me in the viewer. Only in the composer, it's blue.

Yes, you can change the styling via stylesheets. There are some docs on 
my site <http://www.bucksch.org>, but they are outdated, e.g. all 
classes have a "moz-" in front now, e.g. "moz-text-flowed".
