Pretty simple, really: that's how every Mac app (that I've ever seen)
behaves, with the single annoying exception of Mozilla.

You aren't. That's the whole idea. Just as when you're hammering some
nails, you don't need to know (quickly) what brand of hammer or brand of
nail you're using. It's completely irrelevant to the task.
If you really need to, you can always expand the application switcher in
the menu bar to show the application name -- whereupon the duplicate
`Mozilla' in each window's title bar stops looking merely silly, and
starts looking downright stupid.

Again, the icon should represent the document, not the application --
especially since you should be able to drag the icon to a Finder window
or to the Desktop to save the message as an individual file.

Right, so in that case the title should become the name of the folder,
and the icon should become the icon for the folder. Just like in the
Finder or in Windows Explorer.
