The support should be part of the job, and the exact details of the
off-hours support provided (response time, whether or not you will drive
into the office at 3AM, etc) should be negotiated as part of the SLA.

The pay for off-hours support should not be for providing the support, but
for rearranging your personal life to be available.  If you are not going on
vacation (staying close to the office), kicking the wife out of bed and
putting your pager on her pillow, etc your personal life is being severely
affected.  That's what the pay is for.
