Well sorry but my version of the form is in Word, and it is quite a general
format, if I may say... Everybody using Windows has Word, or at least
WordPerfect (which can read Word documents). And it wasn't that big... only
35k or something like that... I wouldn't know which other file-format to use
(except for plain text, but it's hardly possible to edit any layout like
even bold, and forget about boxes etc).
Well... sorry if it bothered you that much... If you want a usable form you
can also use the form on the website.

I don't understand...??? I have an official version and it reads the file
fine. It also can be read in Word 97 (although the links give some trouble
but that's not really problematic in case you want to fill it in).
