Lots of good info here:
ftp://download.nvidia.com/XFree86/Linux-x86/1.0-6629/README.txt
Appendix I and J will be of particulr use
Modelines are a pain though, I recently gave up on trying to get my
DVI to HDMI cable output working on my Panny HDTV and went back to
SVideo output as I could not get a satisfactory modeline other than
720x480 which was not what I was looking for.

Good luck
