Hi Pam, I'm only guessing here, but it sounds like the bank is asking for a DBA. It stands for "doing business as", and as the bank officer tried to explain, it's protects you. Not just for the reasons they mentioned, but what if someone else started using your business name, and they were not up to par with your quality of work/customer service? Having the DBA would give you legal grounds to prevent them from using your name. Of course, you would have to legally go after them to change their name, but at least you would have the option to do so. Or it may prevent someone from using the name to begin with.

I had brought my vendors lic. , but the lady said that that only proves I pay taxes and that I had never "registered" my business name in Ohio ! WTH is this!? I asked her why I would register my name after 20 years of doing nails under this name. She gave me an example of a bank client who didn't register his name, ex. Ohio Carpet Cleaning, and when a competitor opened a business under the same name and REGISTERED it with Ohio, the original business owner had to CHANGE his business name, repaint his signs, his trucks, re-do his business cards, etc. Of course, there is a $50 fee to "register" your business name with Ohio! This is all news to me, what do ya'll think? When I register, is it going to bring an avalanche of paperwork down on me? What changes besides the fact that no one else can use your name?
