  For everything I've seen, with VZW, SIMs are used only to provide
service (billing account) for the GSM radio.  They're not used to
store contacts, and they don't effect the CDMA side at all.  You still
have to call VZW customer service to switch devices.  That is usually
a pretty easy process, just identify yourself and give them the new
ESN.  Processed almost instantaneously every time I've done it.  The
crummy part is, if your device goes dead, your contacts are gone, too.

  VZW offers a service to back-up your contacts for you -- for $2/month.
