One thing I'd add is that you shouldn't try to hire offshore when you
are busy. It sounds whack because the only reason you need the extra
help is because you are busy. But actually it takes a lot of managing
and you just don't have time to do this properly when you are flat out.
The reality is that the default result, unless you do an exceptional job
of managing, is project failure.
I have had mixed results personally. All credit to Paul for making it
work, but I reckon there are easier and less stressful ways to make money.

how to spec a good project, how to select good developers and then to
manage the project effectively.

bad devs and cling on to the good ones. Set clear timelines and
deliverables and don't be afraid to cancel a project if the dev can't
meet the timeframes they agreed to or if the code simply isn't good
enough (or they're not sticking to the requirements).
It's unsurprising to hear the "tried it once and it didn't work"
comments - you can apply that to almost any scenario in life but the
reality is that if you structure your work well, communicate well and
stick with it, outsourcing can be rewarding, effective and profitable.
