Well, Bastion's already been mentioned. Mongoose's Quint Fighter has a system as well, one that exchanges the multiple for Critical Hits and replaces them with specific effects. A little harder than the numbers listed in the DMG for such, but with a short duration and possibility of the wound being perminant.
