The two files are exactly the same, apart from the server name and server Admin

 From the screenshot it looks like your apache config file is missing the line

   Alias /static/nmis/ "/usr/local/nagios/nmis/htdocs/"

towards the top of the file.  Compare your one with the one provided in 
/usr/local/nagios/installer/apache_proxy.conf to see if there is anything else 
missing
