I spent some time with Nancy in Boston in 1997, a wonderful woman. She is now a midwife in private practice.  Recently in an email she was griping about how she was ignored by the obstetricians at a conference, they wouldn't let her speak,  and that "they" were using the term as tho it was their own invention.  Wouldn't believe she had coined it. Does that sound like a familiar story?
