No progress bar, just the spinning pie wheel is normal.  Before you hit
the "Initialize" button, click the "Custom Setup" button on the left. 
Select "Mac OS Extended" from the dropdown, then click "OK" to return to
the Initialize option.  Depending on processor speed, low-level
formatting 4.5 gigs could take from 15-45 minutes.  Since you've had
problems with the drive and have aborted at least one initialization
attempt already, I'd strongly recommend low-level format at this point. 
Start the process and find something else to do for about 1/2 hour.
