I wouldn't rule out a bad card, but chances are it's something else.  My
first suggestion is be sure the card is REALLY seated properly.  Sometimes
it feels like it's in all the way, but it isn't really.

If I recall correctly from when I installed my Sonnet Crescendo G3/400, the
order in which you press the CUDA switch and install the card is important.
I can't remember which one you do first, but check your documentation for
that.

And yes, pressing the CUDA switch will reset your date/time to 1956 as well
as resetting your Appletalk connection to the default "printer", and a
couple other things I can't remember right now.
