Further research indicates some sort of extension conflict. I changed 
extensions sets to "Mac OS9.1 All" and got a good boot with CD drive 
working properly. Even "CD Audio Player" works as designed. Now begins 
the long process of adding back extensions one at a time until I find the 
culprit!
