Hi, thanks for the info. Mine has the black extension. It has the exact
markings you mentioned. On the sticker it says

29089 TELEL
M333/166  1.0
So it sounds like a G3 - 333? Unfortunately, when I try to boot the 9600, it
gives  the startup chime, but no video comes on. The battery is dead in the
9600, but it boots fine from the OEM card.
When zapping the PRAM, it won't do multiple chimes.
Do you think this card is dead or is there something I can do.
