It's unlikely, true, but it *is* possible.  I've actually come close
to it, although only once.  Either I hadn't fully clicked the lens into
position during a lens change (swapping lenses between primary and backup
body), or I managed to push the unlock button while picking up the camera.
But I did have my A* 300/2.8 disconnect from the body while I was carrying
them.  Fortunately the body was on a strap wrapped round my wrist, and I
had a firm grip on the monopod attached to the lens, so no harm was done.
