Dave, my system is as follows: I have two main folders - K10D (where I store unsorted stuff, which of course can be called Unsorted, but I am too lazy to rename it) - Digital (where I store the sorted stuff)
It goes like this: K10D\YYYY-MM-DD - short description Digital\Travel\Norway-2006\YYYY-MM-DD - day 1, etc All my *istD files are called IMGPxxxx.PEF. Since I intend to cross the 10,000 clicks barrier with my K10D I rename the PEF immediately after I copy them from the card to the HD like this IGP0xxxx.PEF where once I get past 9999 it will be IGP1xxxx.PEF etc. I have another directory for Galia shots where I rename the files to _GL0xxxx.jpg although it is unlikely that she will be as prolific. But all this becomes essentially irrelevant when I import the files to the LightRoom where the browsing is very convenient - by camera, lens, date, etc. As for backups I use the EMC Retrospect utility that came with the external hard drive that I have for backups. And I don't have to worry about the files, etc, 'cause this what Retrospect does for me. All I need to tell it is which directories to back up. The restore procedure (I had to restore few files few times ;-) ) is also extremely convenient. Hope that gives you some ideas and/or hints as to how to sort your issue out. Cheers.
