What I don't understand, given what you just said, is why SA is still
used everywhere and dspam is still quite rare? Almost all the mail
applicances I've seen are SA based, and those that do support dspam
(none of which I've got working yet) support it as an option to replace SA.

It's a bit like MySQL/PostgreSQL I suppose; almost everything supports
either MySQL or both, despite widespread opinion suggesting that PGSQL
is better.
