I had a basically non functioning macroadenoma removed by Dr. Shahinian in 2000. (It actually produced very small amounts of LH and TSH.) Before I chose to go to Shahinian Kaiser Permanente (my HMO) told me they had a 50% chance of removing the tumor. They were going to use the traditional transphenoidal technique, which is what is done still in most operating rooms (an incision is made inside the upper lip, an ENT gets the nose out of the way, a surgeon removes the tumor either with a microscope or endoscope and the ENT puts the nose back together.) As invasive as this procedure is, a renowned surgeon like Dr. Laws, can do this procedure and the patient recovers quickly with few complications.) Dr. Shahinian uses a fully endoscopic method where he goes through the right nostril to gain access. ( I encourage folks to join the Pituitary Network Association and read their manual...it gives very straight forward information about the various procedures so you make an informed choice. The manual if worth far more than the $40 membership fee). This procedure is much less invasive and I believe more likely to remove the total tumor. I have heard nothing but accolades about Dr. Shahinian and he did totally remove my 2.2+ cm tumor which had grown outside the sella tursica and was sitting along my brain. I have also heard wonderful things about Dr. Jho who invented the fully endoscopic method. I would like to hear from patients who have had the fully endoscopic method from other surgeons. So far I am not aware of any other surgeons using this method who are getting lots of good reviews. The skill of the surgeon is CRITICAL to a good outcome. Whether they are thousands of miles away and not covered by your insurance (as was my case), I would fly to the best surgeon all over again. The more I learn about the complications that patients experience who do not go to the best, the more privileged I realize I was to have the best. You can call me if you want 541-757-9663.
