We get to a panic in vfs_mountroot, even machines that aren't affected by that bug. Why do I have a feeling that IPS and image-update is broken and not the repository and will take to 10+ emails to get it resolved (look at the length of this thread).

There were many, many kernel boot-issue related bugs in recent builds after build 118. I'm not sure why you're so quick to conclude that it is pkg(5) specific.

Did you check this bug as well?

The same bits when BFU'ed is able to reboot the machine. So, the problem is either in creating the repository (I do make 120/redist_import, changes at http://zhadum.east/export/ws/am223141/nwam-ips-changes/) or image-update.
