This changeset is based on further discussions with Bart and past ones
with johansen.
Change Summary:
* Added an image state check at each major point of plan operations to
determine if plan is still valid. If image state has changed since plan
was created, a InvalidPlanError exception is raised.
* Fixed a bug with catalog.update_entry() not setting last_modified
* expanded unit tests
* changed plan methods to dump in-memory image state at start of
planning to ensure most recent state information is loaded from disk
before planning begins
