I'm also trying now to come to terms with the second case in your analysis
in the bug -- allowing a held package to move forward if another package
with the same hold value was on the commandline.  Is there a utility for
that?  Say we have two otherwise disjoint incorporations that happen to
share the same hold value.  Then upgrading one would upgrade the other.  Is
that what we intend?

And if I can't come up with a good use case for that scenario, I'm pretty
sure that I can't think of one for the multi-valued case, which would allow
an incorporation to release multiple other incorporations, regardless of
their own hold values, when explicitly upgraded.

Well, entire needs to relax all consolidation incorporations, but those
still need to work if entire is uninstalled ....
