expanded description of the Plucker format available

I've expanded the definition of the Plucker format to document the
PalmOS-specific headers.  Please review the draft at
ftp://ftp.parc.xerox.com/transient/janssen/DBFormat.html, and send me
word of any of the no doubt numerous omissions and mistakes :-).

This is part of registering it with IANA as a media-type.
