After two and a half days of pain, I finally got remote printing to work. 
I used webmin to configure the printer and I just enabled the "Printing
Enabled" button.  But I really do not know what made it work.  The printcap
out put was basically the same with just the layout changed.  I did notice
though that webmin changed somethings in the /var/spool/lpd/ directory
(maybe some file and directory permissions. Their time stamp changed ). 

Any ideas as to why it worked now?

BTW, RH7 it uses LPRng so the file to edit to enable remote hosts to print
on the server iis lpd.perms
