   Just to add insight on this feature, some real life
problems I can think of and remember from reading
mailling lists. Spammer uses  a forge From header 
 (eg.  Sexy stripper <[EMAIL PROTECTED]> likot is a
valid email in yahoo.com), Slow or down hosts may
cause DOS?!?! (both sides)  or may lose legitimate
important emails, and this anti spam feature will fail
on all stock qmail  installation ( be liberal on what
you accept attitude or accept now bounce later,  good
or bad not the issue). 
   I'm not saying this feature is wrong or anything
just adding insight as you said they put alot of
thinking on this feature, if you are looking for other
spam prevention tricks, one implementation from
qmail-ldap ( www.nrg4u.com ) worth mentioning is ,

 This file contains blocked addresses for server
without a PTR RR.
 This is useful to stop some spammer that use fake
hotmail or yahoo addresses but mail from home-pc
without a PTR RR dns record.
 Example: @hotmail.com
