No, this is the same as in iPeng. And actually also exactly the same as
on the Squeezebox: Just hit the track you want the playlist to start
with.
The "Play Shuffled" is just an additional option

4.2.1 is the worst update I've seen in a while. Maybe since OSX 10.5.
Surprisingly it looks much worse to me than the 4.2 they originally
distributed to developers as the "gold code" for eventual release. I
suspect they found something significant in there and while trying to
fix it in a week they broke a lot of stuff to the left and right.

That's one of those. 4.2.0 didn't have this. Turning off notifications
fixed it for me.
