It sounds like it is some program that starts, installs something that 
interferes, and that something does not go away when you end the 
program.

But it could be other issues related to Vista that I just don't know 
about.

As an experiment, try rebooting and ensuring no programs start except 
for standard system services.  Ensure bar appears.

Then start one of the programs you work with and no others.  Test if 
bar appears right after starting the program.  Work a bit with the 
program, continually retesting the bar.  Try maximizing and minimizing.

If all tests are successfuly, close the program and retest.

Then try again with another program.

I should also mention that there are times, fairly rare, when even on 
XP PowerPro seems to lose its "stay on top" setting with respect to 
some programs.  This is a Windows bug AFAIK.  Possibly you are seeing 
the same thing.  But it sounds too prevalent to be that.
