1.   First, do you have a similar provision? Yes, we conditional release oil 
samples that are taken to an on-site Lube Lab located outside of the Protected 
Area (Radiological Restricted Area) for analysis.  The samples must clear a 
tool monitor and a blue label identifying the contents and controls is affixed. 
 Transport to the lab is controlled under an RWP and storage and security of 
the oil in the lab is controlled in accordance with 1801 and 8102 requirements.
2.   Do you require completion of a log entry? No
3.   If so, is the log or logbook retained as a formal record and eventually 
sent to records management? N/A
4.   If not a formal record, how long to you keep the logbook (perhaps at the 
RCA exit)? RP does not maintain any formal records; however, the Lubrication 
Section responsible for obtaining and analyzing the samples are controlled and 
tracked under the work order process.

San Onofre has a provision in our radioactive material release procedure for 
"conditional release" where certain items can be escorted from one RCA to 
another if specified requirements are met (such as no loose surface 
contamination, qualified escort, labels if required, etc.)   We currently 
require the completion of a line in a tabular log that identifies the item, the 
escort, destination, etc.

My questions are:
1.   First, do you have a similar provision?
2.   Do you require completion of a log entry?
3.   If so, is the log or logbook retained as a formal record and eventually 
sent to records management?
4.   If not a formal record, how long to you keep the logbook (perhaps at the 
RCA exit)?
