I've just had the same thing happen on a friend's iBook. It turned out to be
bad RAM, which had slowly degraded. This happens, and seems to result from a
static zap at some point which didn't knock out the chip at first but has
slowly degraded its performance over time.

Try removing all non-Apple (i.e. factory-installed) RAM and running diskutil
again. If it gets through the process, put the RAM back in slot-by-slot and
repeat the diskutil test until you find the dud.

Get, and wear, an anti-static wrist strap.
