The fees are set every year for a given health service through the Canadian
Medical Association and the various provincial Governments.

Most doctors have as many patients as they can handle and are happy with the
dollars they receive. If they want more money, they generally let it be
known that new patients are welcome. Most doctors have hospital privileges
and are on call at times to deal with emergency room situations. They may
get new patients as a result. Some doctors have set up clinics that are open
beyond the normal 9 to 5 office hours. They may get new patients this way as
well. 

An interesting comment my brother-in-law doctor made was that all doctors
should be on salary and agree to work a fixed number of hours a week. He
agreed that outside office hours would have to be covered by all doctors.
