If it is spamassassin or clam, can't you temporarily disable those by
adusting the /var/qmail/control/simcontrol ? I don't know since this
issue is well past my knowledge.

You can, but I'm not sure what that would buy you. The problem does
appear to be scanning related. I think it's odd though that incoming
mail apparently processes just fine, while outbound/submissions are the
problem.
I'm wondering now if there isn't a problem with a queue or the
/var/qmail/simscan directory. Glen, is /var/qmail/simscan empty? It
should be except for messages being scanned. Also, what does "qmHandle
-l" show?
