what, in your opinion, are those "real-world" problems exactly? 

I agree with some of your points even though I wouldn't phrase them as
provocatively as you have - I would welcome a more comprehensive and focused
marketing and reaching-out approach. For once, I assume that the team has to
do a lot of in-house development and cannot set all their priorities as they
themselves would like to. After all, this is a company paying people to work
on an open source framework that - it itself - does not make any money for
the company. 

But let's be constructive: What would be the things that development team
should change to increase community involvement? For example, are feature
requests from active members of the community the stuff that will broaden
the audience? Or do you mean 1&1 should hire a graphic artist and create a
couple of sexy themes so qooxdoo can be competitive? I certainly don't have
any answers to that.
