Try winding the tempo right down, to the ridiculously slow value of 10 bpm. Then try it. Does the metronome work then?

Yes. From t=25 and lower it works pretty reliably, although with t=25 it might skip the odd beat, but very rare. However it does work pretty much always for, say, t=10. For normal values, it works very erratically, maybe 1 beat in 10 goes through.

Try increasing the tempo while your piece is chugging along and see when the metronome drops out.

It doesn't drop out all of a piece, it becomes more and more likely that beats won't go through, but even at high values the odd beat will be heard.

You could also try a similar thing by playing very short notes in the drum track, enter notes that for successively short, from 10 ticks downwards. See if they cease to play.

Well, I've done a test with very short drum notes, and they don't play always, they play erratically too.

Also, try changing the key that the metronome uses (Options, Metronome dialog): do all notes have the problem?

I haven't checked this sufficiently yet. It seems perhaps some do more than others, but all do to some extent?

Thanks for your suggestions, at least they seem to be fairly solid circumstancial evidence that the lenght of the notes is playing a role, probably together with the actual time the event is on.
