Oops, I missed something! in your screen shot it actually looks
different from mine. It should look like this 

TCP Any -> 3483
Any -> 9000
Any -> 9090
Any -> 9092
UDP
Any -> 3483

So go in and edit your rules on the router and change your "Source
Ports:" from a "Single" port to "Any."
Now that I think about it though.  I'm pretty sure I could connect to
mysqueezebox.com through the radio before I did any port forwarding.
Maybe I'm wrong. 

I did my port forwarding a little different than you.  I used a static
ip for my computer and then opened ports in router firewall to my
computer's static ip address.  That way worked for me but now I changed
my rules in my router to match yours so the ports are open to the
squeezebox ip and that way works too but PFPortChecker shows my ports as
closed now on my computer. I can still connect to mysqueezebox.com from
the radio though.
