And medicine isn't as black and white as we like to think. An injured pitcher - and I think most major league pitchers with innings on them are injured to some extent - can try to pitch through injuries and hope that things improve with exercise and rehab. Unfortunately, some reach a point where they simply won't get better without surgical intervention, but it takes time to reach that point.
