I follow a simple rule - file it as it's written on the cover spine. 
In terms of folder organisation I find the following works well:
0-E/
F-J/
K-O/
P-R/
S-T/
U-Z/
playlists/

/artist - album/*.flac inside of above

FLAC files are also named "artist - album - track - title" inside of
each folder.

Easy to find anything with or without SqueezeCentre.  I'd originally
also gone for a folder per artist, but then found that browsing to an
album in your filesystem is a pain in the proverbial, which caused me
to switch to the abovementioned.

Most of these packages will scan all file types everytime they're
accessed, but also allow you to exclude certain file types from this
scanning.  The settings are generally found under preferences/ options.
