I have had this happen to me - kind of. I had a mispelling in a rip, and
I ended up with entries for:

Tchaikovsky, Pyotr Illyich (two Ls)
Tchaikovsky, Pyotr Ilyich (one L)

I fixed the spelling on the folders, files, tags, all of it, and ended
up after the rescan with this:

Tchaikovsky, Pyotr Illyich (two Ls) (containing nothing)
Tchaikovsky, Pyotr Ilyich (one L) (containing all the stuff the above
entry had)
Tchaikovsky, Pyotr Ilyich (one L) (yes, a duplicate, but containing
what the "two Ls" entry used to have)

And no amount of rescanning or library rebuilding would fix it. I ended
up moving all things Tchaikovsky out of the library folder entirely,
rescanning from scratch, moving them back in, and rescanning from
scratch. That solved the issue...I don't know what triggered it or what
the deal was, but it's fixed now...

My SB3 is now the least expensive part of my stereo. So I have that
going for me. Which is nice.
And let's put it more impressively - my SBB is the most expensive clock
currently sitting on my nightstand.
