I'm leaning to corrupted headers hence the filename.flac which is what LMS does if it cannot read tags or they're absent. If they were my tracks I'd isolate the albums in question and run the id3 & ape tag removal followed by a tag cut/paste and rescan. If they're still problematic then i'd rewrite the FLAC files using FLAC and retag.
