I suffer from the exact same issue! Thankfully it only happens once every few weeks/months, but it does happen just randomly out of the blue. Oddly enough, like in your instance, it only happens when its not being accessed (at night/weekends). I'm running Debian/Apache2/MySQL.
Side note: may I ask what you ran to show your 'env details'?
