I'm pretty sure I remember someone on this list creating a bubble tooltip
widget some time ago - and I believe (as all good tooltips should) it
allowed plopping any arbitrary HTML inside (e.g. the form you're looking
for).

I'll post a link here if I find it. I could be imagining it - but I almost
can swear I've seen one here.
