[AI] sarthak training and employment programme for persons with disability

2010-07-01 Thread sarthak edu
Dear all

Sarthak Educational Trust completes first year of its operations on
July 1, 2010 with an incredible twelve months of learning as well as
implementing educational and work-place solutions for the differently
abled. We are happy to inform you that the active support and good
wishes of people like you, have enabled us to place 70 persons with
disabilities in various national and international companies within
the past one year. Our heartfelt thanks for your valuable support.

On this occasion, we are also releasing a 22-minute documentary about
our work, the hurdles we faced and how they were overcome. Titled,
INNER VISION, the film is directed by well-known film maker Vijay S.
Jodha on a pro-bono basis and is part of a longer film that he has
been working with us for the past fifteen months. Vijay has trained at
New York University and worked with directors Ang Lee and Mira Nair.
INNER VISION drives home the point that even though differently abled
employees require greater sensitivity and longer gestation period at a
workplace, in the long run they are equally, or even more, productive
than their other fellow workers. You can view this three-part film
on–line at:

Inner Vision / SARTHAK Film Part-1
Inner Vision / SARTHAK Film Part-2

Spoken English and Personality Development Course

Course Code : Spoken English and Personality Development Course
Duration : 3 - 4 Months

About Sarthak:

Sarthak is a registered Charitable Trust formed in 2008. The trust is
working for visually impaired and other disabled with its main
objectives as counseling, education, training and employment-hence
leading to total rehabilitation.

About : Spoken English and Personality Development Course

In today’s corporate environment the need for effective communication
has been recognized and accepted. It is understood that communication
becomes effective when the people who interact are able to comprehend
the requirements of each other in a short span of time. Most often the
first link between people is established through verbal communication,
to be followed by online or other ways of dealing. Moreover as it is
believed that the first impressions are often the last impressions,
the goodwill of a company depends on the initial communication links
that are established and later maintained by its employees. So the
face of the company, that is its employees, needs to be skilled not
only technically but also in effective communication methodology.

With growing international avenues both in the industry and services
sectors the companies are required to deal with foreign clients. One
of the main problems they face in such dealings is the language
barriers due to use of different languages in prevalence.
In most overseas dealings it has been observed that English has been
widely accepted as the functional corporate language. The ease and its
widespread use in most nations has led to the need to recognize the
fact that to get benefits in overseas dealings a command over the
English language is a must. Employees must be well versed in the
grammatical and proper usage of English. Knowledge of both spoken and
written English then would become a prerequisite for getting
employment in companies especially those engaged in the BPO industry,
IT industry or dealing with overseas clients.
Spoken English (Learn with fun) To speak English fluently and
confidently with right accent. Learn to be the leader and not mere
participant.

[AI] article 7 : CONJUNCTIVITIS

2009-10-02 Thread sarthak edu
ARTICLE 7: CONJUNCTIVITIS

Conjunctivitis (commonly called pink eye or Madras eyeis an
inflammation of the conjunctiva (the outermost layer of the eye and
the inner surface of the eyelids), most commonly due to an allergic
reaction or an infection (usually viral, but sometimes bacterial.
While this article relates generally to humans, animals such as
canines (dogs) can experience this condition.
 types:

1. Blepharoconjunctivitis is the dual combination of conjunctivitis
with blepharitis (inflammation of the eyelids).
2. Keratoconjunctivitis is the combination of conjunctivitis and
keratitis (corneal inflammation).
3. Episcleritis is an inflammatory condition that produces a similar
appearance to conjunctivitis, but without discharge or tearing.
The viral and bacterial forms of conjunctivitis are contagious and
were first discovered by Scottish doctors.
 Symptoms:
Eyes with conjunctivitis;
Redness (hyperaemia), irritation (chemosis) and watering (epiphora) of
the eyes are symptoms common to all forms of conjunctivitis.
Acute allergic conjunctivitis is typically itchy, sometimes
distressingly so, and often involves some lid swelling. Chronic
allergy often causes just itch or irritation.
Viral conjunctivitis is often associated with an infection of the
upper respiratory tract, a common cold, and/or a sore throat. Its
symptoms include watery discharge and variable. The infection usually
begins with one eye, but may spread easily to the other.
Bacterial conjunctivitis due to the common pyogenic (pus-producing)
bacteria causes marked grittiness/irritation and a stringy, opaque,
grey or yellowish mucopurulent discharge (gowl, goop, gunk, eye
crust, or other regional names, officially known as 'gound') that may
cause the lids to stick together (matting), especially after sleeping.
Another symptom that could be caused by bacterial conjunctivitis is
severe crusting of the infected eye and the surrounding skin. However
discharge is not essential to the diagnosis, contrary to popular
belief. Bacteria such as Chlamydia trachomatis or Moraxella can cause
a non-exudative but persistent conjunctivitis without much redness.
The gritty and/or scratchy feeling is sometimes localized enough for
patients to insist they must have a foreign body in the eye. The more
acute pyogenic infections can be painful. Like viral conjunctivitis,
it usually affects only one eye but may spread easily to the other
eye. However, it is dormant in the eye for three days before the
patient shows signs of symptoms.
Irritant or toxic conjunctivitis is irritable or painful when the
infected eye is pointed far down or far up. Discharge and itch are
usually absent. This is the only group in which severe pain may occur.
Inclusion conjunctivitis of the newborn (ICN) is a conjunctivitis that
may be caused by the bacteria Chlamydia trachomatis, and may lead to
acute, purulent conjunctivitis. However, it is usually self-healing.

Signs:
An eye, red due to acute conjunctivitis.

An eye with bacterial conjunctivitis.
Conjunctivitis is identified by irritation and redness of the
conjunctiva. Except in obvious pyogenic or toxic/chemical
conjunctivitis, a slit lamp (biomicroscope) is needed to have any
confidence in the diagnosis. Examination of the tarsal conjunctiva is
usually more diagnostic than the bulbar conjunctiva.
Allergic conjunctivitis shows pale watery swelling or edema of the
conjunctiva and sometimes the whole eyelid, often with a ropy,
non-purulent mucoid discharge. There is variable redness.
Viral conjunctivitis, commonly known as pink eye, shows a fine
diffuse pinkness of the conjunctiva which is easily mistaken for the
'ciliary injection' of iritis, but there are usually corroborative
signs on bio microscopy, particularly numerous lymphoid follicles on
the tarsal conjunctiva, and sometimes a punctate keratitis.
Pyogenic bacterial conjunctivitis shows an opaque purulent discharge,
a very red eye, and on bio microscopy there are numerous white cells
and desquamated epithelial cells seen in the tear duct along the lid
margin. The tarsal conjunctiva is a velvety red and not particularly
follicular. Non-pyogenic infections can show just mild infection and
be difficult to diagnose. Scarring of the tarsal conjunctiva is
occasionally seen in chronic infections, especially in trachoma.
Irritant or toxic conjunctivitis show primarily marked redness. If due
to splash injury, it is often present only in the lower conjunctival
sac. With some chemicals—above all with caustic alkalis such as sodium
hydroxide—there may be necrosis of the conjunctiva with a deceptively
white eye due to vascular closure, followed by sloughing of the dead
epithelium. This is likely to be associated with slit-lamp evidence of
anterior uveitis.
Causes:
Conjunctivitis is most commonly caused by viral infection, often
Adenovirus. Bacterial infections also occur, and both bacterial and
viral infections are contagious. Viral conjunctivitis does not usually
require treatment (the

[AI] article 7 cataract

2009-09-05 Thread sarthak edu
ARTICLE 7: CATARACT

A cataract is a clouding that develops in the Crystalline lens Of the
Eye Or in its envelope, varying in degree from slight to complete
Opacity And obstructing the passage of light. Early in the development
of age-related cataract the power of the lens may be increased,
causing near-sightedness (Myopia), And the gradual yellowing and
opacification of the lens may reduce the perception of blue colours.
Cataracts typically progress slowly to cause Vision loss And are
potentially Blinding If untreated. The condition usually affects both
the eyes, but almost always one eye is affected earlier than the
other.

A senile cataract, occurring in the aged, is characterized by an
initial opacity in the lens, subsequent swelling of the lens and final
shrinkage with complete Loss of transparency.
Moreover, with time the cataract cortex liquefies to form a milky
white fluid in a Morgagnian cataract, which can cause severe
inflammation if the lens
Capsule ruptures and leaks. Untreated, the cataract can cause
Phacomorphic glaucoma.
Very advanced cataracts with weak
zonules
Are liable to dislocation anteriorly or posteriorly. Such spontaneous
posterior dislocations (akin to the historical surgical procedure of
Couching)
In ancient times were regarded as a blessing from the heavens, because
some perception of light was restored in the cataractous patients.

Cataract derives from the
Latin
cataracta meaning waterfall and the
Greek
Kataraktes and katarrhaktes, from katarassein meaning to dash down
(kata-, down; arassein, to strike, dash
[3]).
As rapidly running water turns white, the term may later have been
used metaphorically to describe the similar appearance of mature
ocular opacities. In
Latin, cataracta had the alternate meaning”
Portcullis,

So it is also possible that the name came about through the sense of
obstruction. Early Persian physicians called the term nazul-i-ah, or
descent of
The water—vulgarised into waterfall disease or cataract—believing
such blindness to be caused by an outpouring of corrupt humour into
the eye.

In dialect English a cataract is called a pearl, as in pearl eye and
pearl-eyed.
Causes

Cataracts develop from a variety of reasons, including long-term
exposure to ultraviolet light, exposure to radiation, secondary
effects of diseases such
As diabetes, hypertension and advanced age, or trauma (possibly much
earlier); they are usually a result of
denaturation
Of lens
Protein.
Genetic
Factors are often a cause of
Congenital
Cataracts and positive family history may also play a role in
predisposing someone to cataracts at an earlier age, a phenomenon of
anticipation in pre-senile
Cataracts. Cataracts may also be produced by
eye injury
Or
Physical trauma.
A study among
Icelandair
Pilots showed commercial airline pilots are three times more likely to
develop cataracts than people with non-flying jobs. This is thought to
be caused
By excessive exposure to radiation coming from
Outer space.

Cataracts are also unusually common in persons exposed to infrared
radiation, such as
glassblowers
who suffer from exfoliation syndrome. Exposure to microwave
radiation can cause cataracts. Atopic or allergic conditions are also
known to quicken the
progression of cataracts, especially in children.

Cataracts may be partial or complete, stationary or progressive, hard or soft.

Some drugs can induce cataract development, such as
Corticosteroids and Ezetimibe Seroquel.

There are various types of cataracts, e.g. nuclear, cortical, mature,
and hypermature. Cataracts are also classified by their location, e.g.
posterior (classically
due to steroid use and anterior common (senile) cataract related to aging).
Symptoms

As a cataract becomes more opaque, clear vision is compromised. A loss of
visual acuity
is noted.
Contrast sensitivity
is also lost, so that contours, shadows and color vision are less
vivid. Veiling glare can be a problem as light is scattered by the
cataract into the eye.
TREATMENT
contrast sensitivity test should be performed and if a loss in
contrast sensitivity is demonstrated an eye specialist consultation is
recommended followed by surgery.

[AI] article 6 bad breath

2009-08-25 Thread sarthak edu
ARTICLE 6: BAD BREATH (HALITOSIS)

In medical terms bad breath is otherwise known as Halitosis or oral
malodor. Most commonly referred to as bad breath or sour smelling
breath, this is a disorder that arises from diseases of the gums or
the tooth. Sometimes it can also result from intestinal disorders.
This at times may be a temporary phase and it may go after some time
when you follow proper dental hygiene. In case that you find that your
problem persists then you should consult your doctor or if you think
it is a gum related disorder, then you should consult a dentist.

What causes Halitosis? Any food particle that decays produces a sour
smelling stench. The food particles may settle in depression in the
teeth or the particles may remain on the tongue or stick tot eh roof
of the mouth. The odor may also be caused as a result of diabetes, gum
diseases, drugs, and infection in the liver or kidneys, poor diet,
smoking stress and tooth decay.

How to Lower Risks of developing Halitosis? It is quite necessary to
follow a few simple precautions and rules to keep halitosis at bay.
They are (a) Brushing of your teeth twice a day is necessary. Follow
it up with some vigorous flossing. (b) Make use of a good toothbrush
and replace them one in every two months. (c) Do not make use of
mouth washes that have any flavoring in them, this increases the
chance of your mouth stinking in the morning. (d) Stress in one main
contributing factor towards mouth odor so one must try to avoid stress
to the maximum. Get some good exercise and some sunlight on your face.
 (e) Try to avoid foods that stick to your mouth after you have them;
this so mainly for dinner or a late supper. Foods like sweets, meats
and cheese stick to the roof of your mouth and this will bring out the
bad smell in your mouth.

Halitosis is kept at bay by the presence of saliva in your mouth. The
saliva washes away the food particles and keeps your mouth clean. As
saliva produces in the mouth can wash away the halitosis, the
production of saliva is brought down when one is asleep. As the food
particles that remains in your mouth can cause these problems, brush
your teeth well before going to bed.

[AI] article 4 retinitis pigmentosa

2009-08-08 Thread sarthak edu
ARTICLE 4: RETINITIS PIGMENTOSA

Retinitis pigmentosa (RP) is a group of genetic conditions. In the
progression of symptoms for RP, night blindness generally precedes
tunnel vision by years or even decades. Many people with RP do not
become legally blind until their 40s or 50s and retain some sight all
their lives. Others go completely blind from RP, in some cases as
early as childhood. Progression of RP is different in each case. RP is
a type of progressive retinal dystrophy, a group of inherited
disorders in which abnormalities of the photoreceptors (rods and
cones) or the retinal pigment epithelium (RPE) of the retina lead to
progressive visual loss. Affected individuals first experience
defective dark adaptation or nyctalopia (night blindness), followed by
reduction of the peripheral visual field (known as tunnel vision) and,
sometimes, loss of central vision late in the course of the disease.
Diagnosis
The diagnosis of retinitis pigmentosa relies upon documentation of
progressive loss in photoreceptor function by electroretinography
(ERG) and visual field testing. The mode of inheritance of RP is
determined by family history. At least 35 different genes or loci are
known to cause nonsyndromic RP (RP that is not the result of another
disease or part of a wider syndrome).
Genetic counseling depends on an accurate diagnosis, determination of
the mode of inheritance in each family, and results of molecular
genetic testing.
Associations
Retinitis pigmentosa is seen in a variety of diseases, so the
differential of this sign alone, is broad. RP combined with
progressive deafness is called Usher syndrome.
RP combined with opthalmoplegia, dysphagia, ataxia, and cardiac
conduction defects is seen in the mitochondrial DNA disorder
Kearns-Sayre Syndrome (aka Ragged Red Fiber Myopathy)
RP combined with retardation, peripheral neuropathy, acanthotic
(spiked) RBCs, ataxia, steatorrhea, is absence of VLDL is seen in
abetalipoproteinemia.
Other conditions include neurosyphilis, toxoplasmosis(Emedicine
Retinitis Pigmentosa), abetalipoproteinemia, and Refsum's disease.
Treatment
A new treatment that has been shown to delay and potentially reverse
blindness received CE Mark approval in the European Union. The
first-of-its-kind therapy, developed by ScyFIX, uses microcurrent
neuromodulation to help prevent vision loss and even restore sight for
patients with low-vision blindness-related diseases. This is the
first-ever device therapy approval for Retinitis Pigmentosa. ScyFIX
clinical data was presented at the ISCEV (International Society of
Clinicians in the Electrophysiology of Vision), the Retina Society,
and the American Academy of Ophthalmology during 2008. Sixty-three
(63%) of RP patients expanded their visual fields within six months of
therapy. Patients also improved visual acuity to the point that they
are now able to read newspapers and use mobile phones--this was not
possible prior to receiving the therapy from ScyFIX's FDA trials which
have been in progress for more than two years.

Genetics
Retinitis pigmentosa (RP) is one of the most common forms of inherited
retinal degeneration. This disorder is characterized by the
progressive loss of photoreceptor cells and may eventually lead to
blindness.
There are multiple genes that, when mutated, can cause the Retinitis
pigmentosa phenotype. In 1989, a mutation of the gene for rhodopsin, a
pigment that plays an essential part in the visual transduction
cascade enabling vision in low-light conditions, was identified. Since
then, more than 100 mutations have been found in this gene, accounting
for 15% of all types of retinal degeneration. Most of those mutations
are missense mutations and inherited mostly in a dominant manner.

Prevention
The progression of the disease can be reduced by the daily intake of
15000 IU (equivalent to 4.5 mg) of vitamin A palmitate.[13] Recent
studies have shown that proper vitamin A supplementation can postpone
blindness by up to 10 years.[14] Scientists continue to investigate
possible treatments. Future treatments may involve retinal
transplants, artificial retinal implants,[15] gene therapy, stem
cells, nutritional supplements, and/or drug therapies.
New researches
Scientists at the Osaka Bioscience Institute have identified a
protein, named Pikachurin, which they believe could lead to a
treatment for retinitis pigmentosa. In a study published in the
journal Nature, researchers working with mice at the University
College London Institutes of Ophthalmology and Child Health and
Moorfields Eye Hospital, transplanted mouse stem cells which were at
an advanced stage of development, and already programmed to develop
into photoreceptors, into mice that had been genetically induced to
mimic the human conditions of retinitis pigmentosa and age-related
macular degeneration. These photoreceptors developed and made the
necessary neural connections to the animal's retinal nerve cells, a
key step in the restoration of sight. Previously it was

ARTICLE 3: DISEASES OF THE EYE

Astigmatism The image falling on the retina is distorted because the
curvature of the cornea is not the same in all axes.

CataractClouding of the lens, causing decreased vision.

Chalazion Small, hard, cystic mass on the eyelid; formed from chronic
inflammation of a sebaceous gland (meibomian gland).

Conjunctivitis Inflammation of the conjunctiva.

Diabetic retinopathyRetinal effects of diabetes mellitus include
micro aneurysms, hemorrhages, dilation of retinal veins, and
neovascularization (new blood vessels forming near the optic disk).

Emmetropia Normal vision

GlaucomaIncreased intraocular pressure results in damage to retina
and optic nerve. Open angle glaucoma is the most common type of
glaucoma, consisting of a persistent elevation of intraocular
pressure. Narrow angle glaucoma is the acute onset of unilateral
ocular pain and visual loss due to sudden obstruction of the outflow
of aqueous humor.

Hyperopia
(Farsightedness)The focus of light rays passing into the eyes lies
behind the retina, due to a congenitally short anteroposterior
diameter of the eyeball.

Hordeolum (stye)A localized, purulent, inflammatory staphylococcal
infection of a sebaceous gland in the eyelid.

Keratitis Inflammation of the cornea.

Macular degenerationDeterioration of the macula of the retina.

Myopia (nearsightedness)the focus of light rays passing into the eye
lens in front of the retina because of a congenitally long
anteroposterior diameter of the eyeball.

Nystagmus It is involuntary rhythmic movements of the eyes, typically
bilateral, due to congenital abnormality, multiple sclerosis or
central nervous system tumor, infection, or hemorrhage, or
intoxication.

Presbyopia Impairment of vision due to old age.

Retinopathy A general term for degenerative disorders of the retina,
Usually accompanied by loss of vision and of the due to
Systemic disease.
Hypertensive retinopathy is degenerative retinal
changes due to impairment of blood supply to the
Retina and choroid in persons with very high, or
Chronic, hypertension with variable degrees of visual
Loss. Diabetic retinopathy is degenerative vascular
changes in the retina occurring in diabetes mellitus,
particularly in poorly controlled diabetes; the principal
cause of legal blindness before age 65.

Retinal detachment Two layers of the retina separate from each
other.

Retinitis pigmentosaProgressive retinal sclerosis, pigmentation,
and atrophy.

Strabismus Abnormal deviation of the eye. Inward deviation is
called esotropia, outward deviation exotropia. If one eye is
consistently affected, central suppression of its image
eventually occurs, with resulting amblyopia (dulling of
vision that cannot be corrected with a lens).

Uveitis Inflammation of any part of the uveal tract. Anterior
uveitis (iritis, cyclitis, iridocyclitis) causes unilateral ocular
pain, visual blurring, and photophobia. Posterior uveitis
(choroiditis) causes gradual loss of vision in one eye, with
minimal discomfort.

[AI] stem cell therapy in the treatment of eye disease

2009-07-19 Thread sarthak edu
Dear friends
Hello
As you know about the research of stem cell in the treatment of eye
disease is going on at full swing. I have decided to transmit the
information about research to you. Today I am starting with very basic
of stem cell. If you have any queries you can write to me or phone me.
In future I am planning to circulate different articles on medical
subject related to disabilities.
ARTICLE 1: BASIC ABOUT STEM CELL

What are stem cells?

Removing the maternal nucleus during nuclear transfer Stem cells are
the master cells of the human body. They can divide to produce copies
of themselves
And many other types of cell. They are found in various parts of the
human body at every stage of development from embryo to adult. Stem
cells taken from
Embryos that are just a few days old can turn into any of the 300
different types of cell that make up the adult body.

Why are stem cells useful?

Because stem cells are so versatile, they could potentially be used to
repair and replace damaged human tissue. In future it is hoped that
stem cells could
Be used to treat and cure a variety of diseases and injuries including
Parkinson's disease, eye diseases like retinitis pigmentosa, macular
degeneration, stroke and diabetes.

Where do the embryos come from?

Embryonic stem cells can be taken from spare human embryos left over
from fertility treatments, or from cloned human embryos developed in
the laboratory.
This can only occur if fully informed consent has been obtained from
the donors. One method for making cloned embryos is called nuclear
transfer. During
This process, genetic material or DNA from a donor is inserted into an
empty egg cell. After the resulting hybrid cell has been 'activated'
(normally using
An electrical pulse), it begins to divide, creating new cells and
forming a cloned embryo.

Dear friends
Hello
I am a dental surgeon by profession.
I completed my dentistry in 1998.
I practiced for six years then my visual impairment happened.
After my visual impairment, this was end of the road for me.
My friend told me that a VI can operate the computer. He took me to
NAB rk puram by their help I learnt the art of computer.
Then I learnt the art of medical transcription. I worked from home as
a medical transcriptionist.
Then I thought of spreading this knowledge to other VI. So I roam around India.
There are only three centers in India. One is in Mumbai, Chennai and banglore.
In the month of April, I visited banglore center. It was excellent.
Their first batch student has already been placed in the companies.
By the help of enable India we are starting sarthak medical
transcription for VI in Delhi. I think in and around north India there
is no mtvi center.
Enable India especially Shanti ma’am is supporting this programme.
Medical transcription companies will be involving in the training
process from first day onwards so employment is not a problem.
I hope you can tell about this programme to everyone who is dedicated to do it.

[AI] what is medical transcription programme

2009-06-04 Thread sarthak edu
MEDICAL TRANSCRIPTION PROGRAMME

 Dear Medical Transcription Student:

Welcome to the exciting and vitally important career of medical
transcription. I hope that you will maintain an eager interest and
excitement about your course of study. Soon, you will begin to
experience the fascination and appreciation for medicine that working
medical transcriptionists have come to enjoy. Because medicine is ever
changing and because people and their problems are never boring, I can
assure you that you will be always learning and interested in your
work.

Medical transcription is both an exacting science and an artistic
accomplishment. It is important to have a combination of skills
including spelling, proofreading, knowledge of medical terminology,
and typing and a firm background in English grammar, structure, and
style. The successful medical transcriptionist has both accuracy and
speed: a broad knowledge of anatomy; and a thorough knowledge of
medical, surgical, drug, and laboratory terms, in addition, it is
important to know how to use standard medical and nonmedical reference
materials.

An exciting career as a medical transcriptionist awaits you.

AN INTRODUCTION TO MEDICAL TRANSCRIPTION

First, let me introduce you to medical transcription and medical
transcriptionists. You will notice that I use the term
“transcriptionist.” A transcriber is the machine, which plays a
dictated report through our headset so that we may transcribe it. It
is not the person who transcribes.

If there’s one thing that transcriptionist are known for, it is our
love for words. This is a common thread in our profession. It is
what gives a kinship, and it one of the things that make us
professionals.

If I were cast away on a desert island, the book I would probably want
to take with me is an unabridged English dictionary, and I could be
happy for years. Transcriptionist can get lost in a dictionary. We
look up a word and we see something else that looks interesting, and
then we look up another word, and so on. Words are truly exciting. I
would much rather have a dozen new medical words all researched and
defined that a 5-pound box of candy. I hope that you too will come to
share this feeling.

THE SPIRIT OF MEDICAL TRANSCRIPTION

Medical transcriptionists are very special people. We are workers who
sign our initials on each finished product; we are craftspersons in a
modern age, we are practitioners of communication, we are the
magicians of medical terminology, the masters of grammar and
punctuation, and our expertise flows through related areas such as
pharmacology and sophisticated new instruments from the operating
room.

We are in key position as a member of the healthcare team, we break
the sound barrier every day, and we are the trained ears who create
sense out of many diverse accents and sometimes just plain nonsense.
We have a surgical precision all our own, a knifelike wit at times,
and our own scalpel-sharp intelligence, we have that attention to
detail, that sparkle, that scintillating compulsion to look things up,
did the patient see a neurologist or a urologist? We look it up in the
patient’s chart. What is the patient’s middle initial, proper name,
zip code, area code? There are crazy days when every second word is a
spelling nightmare, like pipe fitter, Albuquerque, Quonset hut,
vis-à-vis. Hand me the dictionary, please, every day is like a
treasure hunt in an ocean of words. Education, books, words-we want to
know everything. We also have sensitivity, humor, a sense of
responsibility and ethics, and most importantly, we have excellence,
we are professional serving the healthcare consumer-the patient.

Medical transcriptionists know more about a patient than anyone else
the physician. We wade through gallons of urinalysis and rivers of
blood samples, and we know the chemical state of patients as well as
the most intimate details of their social history. Marching across our
view each day are life and death, struggle with disease, social
problems, and sometimes, thank goodness, human comedy. When we
transcribe a birth record, we breathe a silent “welcome on board” to
the new arrival, when a discharge summary becomes a death summary,
often we are saddened, no matter how old the patient, we have heard it
all and transcribed it all-the blood, the pain, the surprising
recoveries, the agonizing losses, the inexplicable reversals, the
courage, the toughness, the incredible human body’s ability to take
punishment, from the blood test to the barium enema, from chemotherapy
to colostomy, we see the whole picture, form the incompreshensive
patient names to patients from strange-sounding places-geopolitical
smudges on maps we never glanced at until recently.

Why do we care? Why this magnificent mania for learning and
excellence? One reason is that we have sympathy-for the sick, for the
elderly, for the patient who cannot speak English, for everyone put on
hold too often and too long and forced to listen to canned, tinny

please ignore and excuse if this has already been posted here.

Enhancements in JAWS 10.0.1151 (May 2009)

The following is a list of items addressed between the final JAWS
10.0.1142 release and the JAWS 10.0.1151 posting. All enhancements
 since the initial public
beta release are included in this release.

? Resolved an issue in Internet Explorer 8 where the JAWS Scripts that
control reading and navigating the address bar with INSERT+A and
 INSERT+A twice quickly
had stopped working. These commands are very useful when you want URL
information without moving your Virtual Cursor from your reading
 position on the
Web page. These scripts continue to work correctly in Internet
Explorer 7 and Internet Explorer 6.
? JAWS correctly reads and speaks the column and field titles in
Access 2007 database tables. For example, when you press INSERT+TAB
 while focus is in an
Access table cell (field), JAWS will now announce all field
information such as the column name, the field contents, and any other
 associated tutor message.
Previously, the column name information was not mentioned, or it was
necessary to press ALT+TAB to leave and then return to the application
 in order to
 hear the complete field information.
? When you use the UP ARROW or DOWN ARROW to navigate by line in the
Virtual Viewer while in Access 2007, JAWS now speaks each line.
 Previously, JAWS would
only speak the line of text when you pressed INSERT+UP ARROW to read
 the current line.
? The 64-bit version of JAWS 10 now correctly tracks focus when
 running Microsoft Windows 7.
? When you select text using SHIFT+LEFT ARROW or SHIFT+RIGHT ARROW,
such as in the File name edit field of the Open dialog box, JAWS now
 announces each
character as it is selected. Previously, JAWS would just say the word
 ?Selected? without announcing the character.
? When composing a new e-mail message using Outlook Express 6, JAWS
will now announce the words ?Edit, type in text? when you move into
 the message body
using TAB or ALT+` or CTRL+`. Previously, JAWS would say ?Edit, read only.?
? When talking in a Freedom Scientific Webinar training session, you
can use the ALT+L command to lock and unlock the Talk key. This
 command now functions
correctly with the Webinar conference software plug-in that you must
install the first time you attend a training session.
? Resolved several HTML issues when using JAWS with Libera System 7
 Framework? solutions software.

 regards,

 ruchir.

 Several months ago, we started
work to allow NVDA to read secure Windows screens
such as the Windows logon screen, User Account Control (UAC) screens,
the Windows lock screen and the control+alt+delete screen. This work
 stalled for
