Unless I'm in a project, I never see the arrow to all open files, because I
have the setting to always open files in a separate window, which is how I
prefer to work. (So many variables to explain our different experiences!)

I think the difference is whether you have one document or multiple documents
per window. The dots Simdude is seeing are in the sub-menu, which lists the
documents in the selected window. If the window has only one document, there is
no sub-menu, and BBEdit only has room for the active-window
