If I remember rightly, whether you get upgrades is attached to how often you
die, as is the health of one particularly brutal
and got upgrades then died after those upgrades
and none of those upgrades were available in the next life.

One of the tricks
One of the tricks for playing the ensign is if you clear a location of
enemies, save and close down the game. Then play later and maybe you
survive clearing out your next combat zone but don't bet the farm on it.
