John, I'm in the computer business and have been for 30 years. I have never seen such a device. However, using something like Arduino Leonardo http://arduino.cc/en/Main/ArduinoBoardLeonardo one could easily create such a thing using a common 3x4 keypad and a fairly small amount of code.
