I think they have done a pretty good job describing this in the 3rd edition of 60950, see 3.1.9. The 2nd edition seems only to regard this as applicable to primary power terminals (3.3.4). This is probably why the agencies had to apply their own interpretations (i.e. UL PAGs) over the years.
