Did all those steps on a test environment, apart from steps #19 and #20,
instead of which powered off the old server.
The clients are unable to connect to their mailboxes using Outlook 2000.

I'm not saying don't ever do it, just don't make it a default. Make it so
that the admin has to manually add his NT account to the box each and every
time. Make your server update the directory perms
Just make sure the forgetful Exec's give you permission in writing. As you
point out, they won't remember they did. Then ponder what they'll think if
a sensitive message from their sent ends up in the wrong hands...THEY know
they couldn't have misclicked the recipient.
And so forth...
Make your ExchangeAdmin NT group the owner of all resources.
Execs should be shot, and they will be the first one to take you to court.
