Thanks to all for the suggestions, all of which I tried.
I still don't know why, but when I removed the book-within-a-book it all
worked fine. I shall investigate further, but have Plans B to Z in case I can't
find a proper solution.

Make sure the GroupTitlesIOM format is specified in the book file PDF Setup
and not just in the Index file. (In the book file, select all files
including the book file, then Format Document PDF Setup Bookmarks tab.)

Don't include the *TOC tags in your generation of PDFs. You can
