I would use the newest version of NC State Board of Health and then note
work
- would I consider using two separate citations.

For inconsequential changes like you mention, I agree with Mary 100%. The
question I ask myself is would someone on the LUG be able to duplicate my
work? If I answer yes, then I go with the condensed version
For me... Each one is a source is it not ? You only have three. I
would source, each one (not 17) but your three you have found, and cite
them accordingly.

David, I disagree with you completely. The internet is a repository, like a
library. Ancestry.com is like a book (or more accurately, like a series of
multi-volume books) within that library. I certainly would cite the name and
page number of a book I found within a library.
While I agree
I agree with Cynthia.
Your source is an image from Ancestry.com, not an official death certificate
from the North Carolina Board of Health (or whatever it may have been called in
the past). Your citation needs to be clear that you were using an online image
from Ancestry.com. You really
I'm coming in a little late on this topic but here's my 2 cents worth.
First, ask yourself will someone else be able to reproduce my work following my
sources? If the anwer is yes, don't beat yourself up too badly about proper
format. Second, don't drive yourself crazy with tiny details.

I don't agree Connie. Nothing is stored on the Internet, but on
servers that you can access thru the internet. The Internet is just a
method to allow you to get to the servers such as on Ancestry.com.
And even at that, Ancestry.com probably does not have the originals,
but only copies. So where
I agree, Mary. I probably wouldn't go to that level of detail regarding the
name of the agency, but if I did, I would add it as a comment as you suggest
and use only one citation format, not three.

I would use the newest version of NC State Board of Health and then note that
other names such as NC Department of Health and NC Department of Health and
Human Services have also been used in the past.
