That's pretty much it Carolyn.
For reference, there are only 3 other municipalities that are
independent of counties outside Virginia -- Baltimore (since 13 May 1851),
Saint Louis (since 1876) and Carson City, Nevada (have to look that one up,
but it happened in the late 20th century).

I don't know the "official" answer, but this is fairly rare (in Virginia, the
local government entity is either a city or a county, but cities are not part
of counties). Using the normal practice of "city,county,state, country", I
would put the "[city name],,Virginia, United States".

Thanks so much for the ideas. I will try them. I always use VA if it is
before West Virginia became a state. Once it became a state I use WV. I
know those counties, so for me it works well. However if you are using GPS
etc. I would put it as WV with (VA) after it. I don't know if that would
