Well my $0.50c why not go for a PDF form.
*IF* the organisation already has a web site then setup a
link so folks can download the PDF on the link page their be
a notice stating something like...
you who fill in the form and email it back to us do so with

I looked into this but from what i read you have to be on
the same circuit or else it is again it and miss with the
connection thro the power lines, I ended up getting a N
class wireless router.

The wireless link drops out randomly or all devices more
often than in the past, powering off doesn't help things.
Looking for thoughts on a replacement.
Usage
Around the 7th I got a reply from card services (had a card
